You the permittee/user are solely responsible for any/all information provided herein.

You the permittee/user are a competent person authorized to initiate requests using this service.

You the permittee/user understand that all permit numbers submitted herein are bound by stipulations which can be found on your original hard copy permit or electronic permit.

It is solely your (permittee/user) responsibility to understand the stipulations binding the permit number you are entering using this service.

You the permittee/user understand that any violation(s) of the stipulations associated with the permit number listed herein will result in fines for all associated stipulations.

You the permittee/user understand that receipt of a confirmation number is not authorization to start work contrary to any stipulations associated with the permit number listed herein.

You the permittee/user must submit this form at least two hours in advance of beginning the restoration of any opening or excavation on a protected street.

In addition during the backfill and compaction phase of the work, permittees must provide, on site, a certified compaction tester from an approved laboratory or a license certified tester to test that the compaction of the backfill is in accordance with the Department’s rules and specifications.
